What Makes SILK ELITA Different from LASIK?

Traditional LASIK creates a corneal flap using either a microkeratome blade or a femtosecond laser, then reshapes the exposed stromal bed with an excimer laser. SILK ELITA (also referred to as the SMILE class of procedures) eliminates the flap entirely. The SILK ELITA J&J femtosecond laser creates a precisely shaped intrastromal lenticule — a thin disc of corneal tissue — which the surgeon then removes through a tiny incision of less than 4 mm. The corneal epithelium remains virtually intact. This architecture preserves the cornea’s mechanical strength, reduces post-operative dry eye incidence, and makes the eye far more resilient to pressure or impact after surgery. Recovery is typically 24–48 hours for functional vision, with best-corrected outcomes achieving 20/20 or better in the overwhelming majority of treated patients.

The EVO Visian ICL (STAAR Surgical) is a phakic intraocular lens — it sits between the iris and the natural crystalline lens, requiring no corneal tissue removal whatsoever. ICL (Implantable Collamer Lens) surgery places a biocompatible lens inside the eye, behind the iris, without touching the cornea. It is the preferred solution for patients with high prescriptions (–3D to –18D myopia), thin corneas that are unsuitable for LASIK or SILK ELITA, significant dry eye disease, or hyperopia and astigmatism outside the LASER correction range. The procedure takes approximately 15 minutes per eye, is reversible, and delivers outstanding visual quality — often superior contrast sensitivity and sharper night vision compared to spectacles or contact lenses.
LASIK and SILK ELITA are performed under topical (eye drop) anaesthesia — there are no injections or general anaesthesia. During the procedure, patients typically experience only mild pressure sensation lasting a few seconds. After Contoura Vision LASIK, functional vision returns within a few hours; after SILK ELITA, most patients see well within 24–48 hours. The minimum recommended age for LASIK and SILK ELITA is 18 years, with prescription stability for at least 12 months being equally important. For ICL surgery, the recommended age range is typically 21–45 years, though the primary criteria remain prescription stability and clinical suitability rather than age alone. Patients above 40 are counselled about early presbyopia — the natural loss of near focus — and how that interacts with post-operative vision planning.
LASIK and SILK ELITA permanently reshape the cornea — the correction applied to your cornea does not change. However, like all individuals, your eye’s refractive status can evolve with age, particularly with the development of presbyopia (after age 40) or cataracts (later in life). The vast majority of patients remain spectacle-free for distance vision for many decades. If a small residual prescription develops years later, an enhancement procedure may be possible subject to corneal thickness suitability.
Contoura Vision LASIK on the WaveLight EX500 can correct myopia up to approximately –8D to –10D (subject to corneal thickness), hyperopia up to +4D to +6D, and astigmatism up to –5D to –6D. SILK ELITA has broadly similar correction ranges. ICL surgery extends the correction range significantly — the EVO Visian ICL corrects –3D to –18D of myopia, with the toric variant additionally correcting up to –6D of astigmatism. For very high prescriptions where corneal LASIK is unsafe, ICL is the clinically superior and safer choice.
Standard wavefront-optimised LASIK corrects spectacle prescription (sphere and cylinder) uniformly across the treatment zone.
